Chronic rupture of Achillis tendon operative management using two in one operation

Abstract 

We report the results of surgical treatment of chronic rupture of the Achillis tendon using peroneus brevius (White and Kraynick) and LARS ligament (ligament advanced reinforcement system). We reviewed 13 patients (all were males) at a mean period of 45 months. The median time from injury to surgery was 3 months (2–4 months). The median age of the patients was 43 years (29–50 years). The median time to full weight bearing was 5.8 weeks (4–13.5) and return to sports 20 weeks (12.3–34). After surgery two patients had skin necrosis. There was no patient with re-rupture of the tendon or injury to the sural nerve. There was small loss in calf circumference, range of motion and plantar flexion strength. Ninety percent of the patients scored over 72 points or higher on the AOFAS scale. The Tegner activity score for all patients decreased from 2.7 to 1.9. Peroneus brevius tendon transfer and augmentation is a good option for treatment of chronic Achillis tendon rupture.
